Living a Healthy Life Workshop Series

Are you living with heart disease, diabetes, high blood pressure, arthritis, osteoporosis or other chronic conditions? Are you caring for someone with chronic conditions?

Overview

If you’re feeling overwhelmed by the daily struggles of living with a chronic disease, know that there is a path forward. Our 6-week Chronic Disease Self-Management Workshop offers a supportive environment where you can learn to navigate these challenges with confidence. By connecting with others who understand, you’ll discover new ways to enhance your well-being and regain a sense of control over your health.

This program helps people who:

  • Live with an ongoing condition like diabetes, arthritis, lung disease or any other condition
  • Experience issues such as chronic pain, fatigue, depression, anxiety or stress
  • Feel “stuck” or that change is difficult

Topics include:

Managing Symptoms: Techniques to deal with problems such as fatigue, pain, sleeplessness, and stress.

Mind-Body Connection: Strategies to improve psychological well-being including relaxation techniques.

Healthy Living: Tips for healthy eating, self-care, and exercise that can improve quality of life.

Action Planning: How to set realistic goals and the steps to achieve them.

Problem-Solving: Skills to tackle the day-to-day challenges of living with a chronic disease.

Decision Making: Guidance on making informed treatment decisions and managing medications.

This workshop is not just about learning; it’s about finding companionship and understanding in a community that cares. Join us to take the first step towards a healthier, more empowered you.

Workshop format:

Participants attend 2.5hr sessions, once weekly for 6 weeks. The workshop is interactive and is limited to groups of 10-15 people.
