About Us

South West Self Management

The South West Self Management Program is housed within the Thames Valley Family Health Team, funded by Ontario Health and serves the communities of Grey, Bruce, Huron, Perth, Oxford, Norfolk, Elgin and London/Middlesex. The goal of the program is to have a coordinated approach to support people living with chronic conditions, caregivers and health care providers with self management support, information and training.

How we help

Individuals living with chronic health conditions spend very little time with health professionals, most of the time they are managing on their own.

What is Self Management?

  • Who is the program for?

    It is for individuals across Ontario who are living with one or more chronic conditions such as diabetes, arthritis, respiratory disease, kidney disease, chronic pain, cancer, or any type of long-term physical or mental health issue. Family, friends, and caregivers of someone who has a chronic condition are also welcome. Individuals looking to improve their health literacy will also greatly benefit from the programs as well as health care professionals interested in enhancing their patient self-management support skills.

  • What does it cost to participate?

    Participation is FREE.

  • Do I need to be referred to the program?

    No. While some people are referred, anyone who is interested is welcome to participate.

  • Does this program replace existing programs I'm participating in?

    The workshops, information and supports provided by South West Self Management do not conflict with existing treatments or programs, and are designed to enhance disease-specific education. The program may be especially helpful to individuals who are living with more than one chronic condition, providing them with the skills and confidence to better manage their health.

  • What do I need for a virtual workshop?

    Access to a computer, iPad, iPhone or android device - you'll need a computer, phone or tablet with video, audio and microphone capabilities in order to participate. Never tried an online workshop before? Don't worry we'll help get you started.  In-person workshops are also available.

By writing down my goals and then being accountable to the others in the group, led to more successes. The breathing exercises have helped my quality of sleep. Reading nutrition labels has led to healthier choices.

Other healthy living workshops I attended focused on a teaching and telling style, and this peer to peer approach was more meaningful for me. Not only did I learn from the book, I learned from the others in the group as we took part in the brainstorming and decision making activities.

The facilitators really walk you through step by step how to utilize the skills in a practical setting and break it down in a way where you are building upon what you learned week after week.
